Hello Mr. Brown, My name is Jane and I am your PA today otherwise known as a Physician’s Assistant. The doctor had ordered some test for you and I am going to go over your results and explain to you what each of them mean. If you have any questions please feel free to stop me at any time. First we will start off with your Triglyceride level and it was 145 mg/dl. Your Triglycerides is fat in the blood and they are used to provide energy to your body and they are the main form of fat in your body (WebMD, 2014). Your levels are considered desirable meaning they are not very high, however we would like to see them a little lower. The second test that was given was to check your cholesterol and your levels were 210 mg/dl and that is considered borderline high. Cholesterol is a waxy, fat-like substance made in the liver and other cells. Mr. Brown if your Cholesterol is too high and it is it can cause heart problems (WebMD, 2014). The last set of test was to check your LDL and HDL le vels. LDL or Low density lipoproteins is known as bad cholesterol because it can cause plaque buildup on the walls of arteries and the more LDL you have in your blood then the great the risk of heart disease. Your LDL levels were 160 mg/dl and that is considered high, a more normal range should be below 130 mg/dl. HDL or High density lipoproteins are known as good cholesterol and it helps the body get rid of LDL. Your HDL levels were 33 mg/dl and that is considered to be poor. If your HDL levels are low then your risk for heart disease goes up (WebMD, 2014). Mr. Brown there are many factors that play a role for having high cholesterol and some of those you can control and others you can’t. For example, the ones you can’t control are your genetics or family history. The ones that are within your control are inactivity, obesity, and an un-healthy diet. There are many treatments for reducing your high cholesterol like changing your diet or medication. I would like to suggest that you change your lifestyle before you are put on regular medication to control your cholesterol. I know this is something that is of no interest to you because you stated you didn’t want to chang e your lifestyle, however, even if you  are prescribed medications you will still have to make a change to the way you are currently living. Let me tell you the side effects of the medication used to treat high cholesterol. I just want to let you know that tolerance of medications vary from person to person. â€Å"The common side effects are muscle pains, stomach pain, constipation, nausea, and diarrhea† (Mayo Clinic, 2014). Mr. Brown I am sure you are wondering if the benefits of lowering your cholesterol outweighs the risk of taking medication and yes they do. Statins are known as a group of drugs that inhibits the synthesis of cholesterol and promotes the production of LDL and decreases the levels of HDL. These pills may cause a small increase in weight gain and blood sugar which can also increase the risk of Type 2 diabetes, however, they will also prevent you from having a heart attack and stroke so in the long run you would be better off to make simple lifestyle changes and take medication. If you choose not to take medication to help control your cholesterol then there are some other options we can look at. To start off you could change your diet because many foods such as eggs, beef, pork, and cheese will raise your cholesterol. You could try to maintain your weight by exercising more, don’t miss annual physicals with your physician, and quit smoking (Health Media Ventures, 2014). Mr. Brown I can reassure you that we can help you get your cholesterol under control. There are many things as I have said that can be done to help lower your cholesterol and help keep it lowered. I know this is a lot to take in at once but I’m not only here as your PA but also as a friend. If you would like additional information then I can send papers home with you so you can get a better understanding of what is going on and how you can control it. There are many options out there for you but you have to take the first step towards making this change. All I can do is be here for you and share my knowledge with you. I don’t want you to think that I am trying to drastically change your life style or make you do so. There are simple things that can be done to help lower your cholesterol and prevent you from someday having a heart attack. So here are some things I recommend you to do in order to lower your cholesterol eat more soluble fiber and that is found in foods lik e beans, fruits, oatmeal, and vegetables. Eat less fat and fewer foods like meats and dairy products, exercise at least 30 minutes a day on most days of the week, and lose weight. Mr. Brown if I were you I would try these simple changes  to begin with and if doesn’t seem like those changes are working then we can try medications to help lower your cholesterol, but like I said before you will still have to interpret exercise and eating healthy into your daily diet. I know it will be hard at first but with the help of family, friends, and health care providers I know you can do this. I want to be honest with you Mr. Brown, it is important that you make these necessary lifestyle changes in order to live a longer healthier life. I’m not sure if you have a wife, children, or grandchildren but wouldn’t they be worth it to make these few changes in your life? The fact is if you don’t make these necessary changes then you don’t have any other options and you are putting yourself at risk for a heart attack or possibly a stroke, then your wife will be left without a husband, your children without a father, and grandchildren without a papaw. Macbeth, a Play Full of Darkness Essay

Macbeth is a play full of darkness, evil, and tragedy. It is the story of a man who goes against his conscience and commits a horrible deed which leads to his destruction and loss of everything he has around him. This includes the relationship he has with his wife, Lady Macbeth. In the beginning we see Lady Macbeth playing the more superior, more dominating role of the two. She comes across as a woman, who is persuasive and manipulative. She seems willing to trample anyone in order to get what she wants. She lays all the plans and all Macbeth has to do is obey her commands. Macbeth on the other hand is fickle-minded and unsure. We discover that the man, who is praised so highly by the public is actually a weak man. We see that he is not the brave loyal man he is portrayed to be in the beginning. Macbeth and Lady Macbeth have a very strong relationship but this later deteriorates. Act 1 Scene 5 is a key scene which shows just how close Macbeth and Lady Macbeth were at the beginning of the play ;it shows their original relationship. Macbeth has written a letter to Lady Macbeth telling her of everything and in this letter states â€Å"my dearest partner† which shows that she helped him and they are in it together. . The following speech where Lady Macbeth doubts that he can get to the title of King â€Å"he is too full of the milk of human kindness† shows just how close they were. It establishes the fact that she knew him so well, she knew what he was like and it emphasises the closeness of their relationship. She speaks of how he has enough ambition but not enough courage. His â€Å"overiding ambition† is not enough. When Macbeth and Lady Macbeth speak, they speak to eachother with such closeness and bond ;he calls her his â€Å"dearest chuck†, his â€Å"partner of greatness†. She knows that he is too weak to do anything and states her position in the murder â€Å"leave the rest to me†. In Act 1, Scene 7 shows the force and power that Lady Macbeth has over her husband. Upon hearing of Macbeth’s decision not to kill Duncan, she is outraged and starts to work her force and power upon him. She knows where he is most vulnerable and attacks him at his weak spot. She strikes him at his manhood and courage. This of course works on Macbeth and she knows that it will. No one calls Macbeth a coward. She says that he is a coward and attacks his manliness. â€Å"to be more than what you are, you would be so much more the man†. She challenges his love for her and says that she would rather â€Å"dash the brains out† of her own child than break such a promise as Macbeth has to her. At this point in the play, Shakespeare re-confirms just how close the relationship between Macbeth and Lady Macbeth is and that she has the power and he listens to whatever she has to say. Lady Macbeth is s major influence on Macbeth, but this of course changes later. After the murder, Macbeth is still carrying the daggers and he seems to be quiet and uneasy. Lady Macbeth has to clean up what he has done wrong and has to return the daggers herself. Lady Macbeth is still very much in control. Here, Shakespeare defines both Lady Macbeth and Macbeth’s strong and weak characters. Lady Macbeth being the strong one yet as a duo, he tones them down to be nothing but two whispering, frightened villains. Later on, when Macduff discovers the body of Duncan, Macbeth acts suspiciously and draws attention to himself. It is then that Lady Macbeth has to help him out and draw the attention away from him and to her by fainting. She does this later too, during the banquet scene. Lady Macbeth is always there to take the attention away from Macbeth. However, towards the end of the play, Lady Macbeth, comes across as one who has succumbed to her guilt. She discovers that nothing that she does could rid her off her guilt, by admitting that even the ‘sweet-smelling erfumes of Arabia’ would not be able to remove the stench of blood from her ‘little hand’. She now takes up the role of the weak, submissive partner in the relationship, who is unsure of herself and very frightened of the future. Macbeth, on the other hand, now makes all his decisions by himself, and reaches the extent whereto he does not even bother to inform his wife of his plans. He gains false confidence from the witches second predictions and builds castles in the air. He becomes a tyrant and a man, despised and hated by his public. He becomes ‘insane’ and goes out of control.

The book consists of a set of articles in which the centre of attention is the notion of Polysystem. The article The Function of the Literary Polysystem in the History of Literature is mostly connected with the notion of Polysystem. Language is heterogeneous, so it is labeled as a polysystem, where highly codified stratification prevails, any minor move from one stratum to another may be taken as a major offense. Polysystem is subdivided into â€Å"canonized† (usually considered â€Å"major† literature: those kinds of literary works accepted by the â€Å"literary milieu† and usually preserved by the community as part of its cultural heritage) and â€Å"non-canonized† literature (those kinds of literary works more often than not rejected by the literary milieu as lacking â€Å"aesthetic value† and relatively quickly forgotten, e. g. detective-fiction, sentimental novels, westerns, pornographic literature, etc. ). Translation plays a great role in Polysystem and in the synchrony and diachrony of a certain literature. Epigonic† literature is kind of a literature where diachronic shifts create a situation in which norms previously known as dominant become peripheral within â€Å"the new phase of literature,† but they are still in use. Canonized literature tries to create new models of reality and attempts to illuminate the information it bears, non-canonized literature has to keep within the conventionalized models which are highly automatized. In synchrony, canonized and non-canonized systems manifest two various diachronic phases, the non-canonized overlapping with a previous canonized phase. The oppositions between the various literary systems create an ideal literary and cultural balance within the literary polysystem. This is the only way for non-canonized literature to settle in literature when canonized literature succeeds in gaining ground. The article The Relations between Primary and Secondary Systems in the Literary Polysystem distinguishres between primary and secondary systems – to what extent a certain system or type plays a major role within the literary polysystem. The Polysystem hypothesis gives us possibility to a more adequate analysis of intra- and interrelations. It is also a functional concept. It is concerned with dynamic complexes. The functions it may detect are conditioned by complex inter- and intrarelations within a hypothesized multileveled system. â€Å"Primary† type: the pre-condition for its functioning is the discontinuity of established models. Change occurs only when a primary model takes over the center of a system. Sometimes we can have the process of â€Å"secondariztion’ of the primary, what means that new elements are retranslated, into the old terms, inconsequence imposing previous functions on new carriers but not changing the functions. The Polysystem Hypothesis Revisited. Many scholars, including Evan-Zohar considered that literature within the historical context can be conceived of as a polysystem, that is a stratified whole, where the various strata function as systems. But it became clear that no literature really overfunctions as a non-stratified whole and if the correlation between the strata within it disintegrates for some reason, a sort of stagnation takes over. The result of such process is that the system collapses. The methodological hypothesis which presumed literature to be a polysystem could now be reversed and reformulated in terms of a universal: all literary systems strive to become polysystemic. The article Universals of Literary Contacts distinguishes source literature (SLt) and target literature (TLt) and draws the difference between two major types of literary contact: first – contacts between relatively established systems which are con-sequently relatively independent (for instance the situation of English and French languages over the past two years); and second – contacts between non-established or fluid systems which are partly or wholly dependent upon some other system(s) – these are the cases with Ukrainian and Russian, for instance. Some examples of possible universals of literary contact are provided: – Literatures are never in non-contact, because of some social reasons. These contacts are the rule rather than the exception. – A prestigious literature may function as a literary superstratum for a TLt. – Literature may be selected as a SLt when it is dominant due to extra-literary conditions, for example the political domination of English and French during colonial periods. – Contacts are also favored/non-favored by a general attitude of a potential TLt. Due to some nationalistic view, works of some writers could not be possible considered as appropriate for the national literature. – We may observe interference when a TLt cannot resist it or has a need for it – is when a certain type evolved in a certain literature is lacking in another, for example, thrillers and detective stories undoubted-ly migrated from English literature to practically all the others. – Items are not transplanted from a SLt to a TLt, but â€Å"needs† determine and guide the selection and the function also changes. – A TLt tends to behave like a secondary system with respect to a SLt. This means that a TLt, under the conditions mentioned above, makes contact with a diachronic phase of a SLt which is wholly or partly outdated, and disregards a contemporary SLt phase. Interference in Dependent Literary Polyssytems. A literature may become dependent only if it is weak. But this â€Å"weakness† means the inability of a system to function by confining itself to its home inventory only and the extreme of such a state would then denote a situation where a literature can function only because it has the opportunity of using some other literature. Russian literature was for Hebrew as a part of it on all levels, but due to the equivalency policy adopted by Bialik and his contemporaries, there was a very strong illusion of domestic â€Å"originality. † When the condition of weakness is not marked, even physical contacts and pressures may not result in interference; when it is marked, a state of dependency is very likely, especially if simultaneously accompanied by accessible contacts even in the absence of physical contact and pressures. The next article Russian and Hebrew: The Case of a Dependent Polysystem first of all tries to show ways in which Yiddish and Hebrew developed through the centuries. The fact that many Jews lived among Slavs is of high importance because this is the reason why the language was influenced by European languages. A test called haematocrit is the testing of the proportion of red blood cells compared to all blood cells in a certain volume. A lower than normal finding on a hemotocrit could mean anemia, a large number of white blood cells due to illness infection Leukemia, Lymphoma or other white blood cell disorders, vitamin or mineral deficiency or recent blood loss. The effect of a bacterial infection on the hemotocrit would cause a decrease of the haematocrit. Compare the development of lymphocytes with the development of other formed elements. There are two kinds of lymphocytes, B lymphocytes and T lymphocytes. Lymphocytes begin in the bone marrow, if they stay there then they are B lymphocytes, if they move to the thymus they are called T lymphocytes. The other formed elements of blood start with a mixture of  plasma that develop through erythrocytes and the platelets are formed by megakaryocytes exploding. What is erythropoesis? Which factors speed up and slow down erythropoesis? Erythropoesis is the process of making new red blood cells. This begins in red bone marrow. Proerythroblasts divide several times until it reaches the end of its development, then rejects the nucleus and becomes an reticulocyte. After being released from the red bone marrow a few days later they turn into erythrocytes. Erythropoiesis speeds up when there is a lack of oxygen to body tissues and slows down when there is sufficient oxygenation throughout the bod y. Explain what would happen if a person with type B blood were given a transfusion of type O blood. If a person with type B blood was given a transfusion of Type O there will be no negative affects. Type O blood is the universal donor as there is no antigen on them that would be attacked by the anti-A antibody on the B type blood cells. During an anatomy and physiology exam your asked to view white blood cells in prepared slides of standard human blood smears. Based on the observations below what is the name and function of each WBC? A.WBC has a round nucleus surrounded by a blue halo of cytoplasm with no visible granules. This WBC is a lymphocyte. The function of a lymphocyte is an immediate response of the immune system to defend against known pathogens ASAP. B.WBC contains dense blue purple granules that hides the nucleus. This WBC is a basophil. Their function is to store histamine and are involved in a response to inflammation. C. WBC has a u-shaped nucleus and a bluish cytoplasm with no visible granules. This WBC is called a monocyte. The function of a monocyte is much the same as the neutrophil, they eat invading pathogens to help protect the body. Monocytes also eat dead and damaged cells to keep the body healthy. D.WBC contain small, pale lilac granules and a four lobed nucleus. This WBC is a neutrophil. The function of neutrophils are to eat bacteria and some other foreign material. The membrane of the neutrophil surrounds the invading bacteria and then destroy it. E.WBC contains red orange green any two lobes nucleus. This WBC is an eosinophil. Eosinophils have a few functions, they are part of the inflammatory process, the trap substances, kill cells, and have anti-parasitic and bactericidal activity. Why would the level of leukocytes be higher in an individual who has been infected with a parasitic disease? Leukocytes help defend the body from infections, as someone is getting sick from a disease leukocytes target the threat to keep the body healthy and to destroy the threat. When a doctor looks at the results from a complete blood count of CBC and see that there is an elevation of white blood cells he knows that there is an infection or immune issue. In regions where malaria is endemic, some people build up immune resistance to the malaria pathogen. Which WBC is responsible for the immune response against pathogens? How do they function? Lymphocytes are responsible for the response against pathogens. First you must be exposed to the pathogen, after that the lymphocytes know what to look for. When you are re exposed to the disease your body is prepared for it and keeps you from getting sick or at the very least from getting the full blown sickness. What is the function of prothrombinase and thrombin in clotting? Explain how the extrinsic and intrinsic pathways of blood clotting differ. The function of prothombinase is to turn into prothrombin in to thrombin to make blood clot, which then stops blood loss. The difference between extrinsic and intrinsic is where the chemical connections comes from. The extrinsic bond happens outside of the damaged blood vessel when blood seeps into other tissue. The intrinsic bond happens inside of the damaged blood vessel. With both pathways after the chemical bond is triggered a blood clot begins to form and stop the bleeding. Democracy, Journalism And Society Democracy is defined, basically as government in which the supreme power is vested in the people. Democracy is a very challenging form for politicians and it can be exercised by the people (Becker Raveloson). According to U.S president Abraham Lincoln (1865), he once stated that â€Å"government of the people, by the people, for the people†. So democracy literally is a set of ideas and principles that used to protect the basic human rights, freedom and to uphold citizenship. And in democracy, media is playing a contributory role by informing the public, educating voters, promoting ideas, and ensuring the government are accountable and transparent. Ever since, media has become a forum for public discussion and public debate has been recognized since 17th century. And today the communication way is still dominated by the media such as television, radio, newspaper, magazine, internet and etc. No matter is old age media or contemporary mass media, the distinctive function is still remain unchanged as media is used to disseminates information and form 2 way of communication (Crozier Huntington Watanuki, 1975). In order to obtain wider range of information, media audiences are replying on media information. For an example, voters watch TV to get the latest update during election campaign. In this case, from my point of view I agree with media is playing a crucial role in shaping a healthy democracy. Media audiences are more aware of various social issue, political and government activities through mass media. Dewey (2008) continue to argue, media also playing as a watchdog to guide of public interest over government and monitoring function. In news journalism media, watchdog is referring to an individual or group of people that come with notion of avoiding illegal practices. This can be seen as one of unique strength in media that helps to shape a healthy democracy because watchdog is ensuring government accountability in democracy and access the government performance. (Coronel, 2003). Watchdog is there to against abuse of power, corruption, financial irresponsibility, assorted malfeasance and other wrongdoing that might threaten the society. Example, based on the case that happen in Thailand, the investigate reports and the press has uncovered wrongdoing of the Prime Minister Thaksin Shinnawatra. ( Miranda, 2002). The strength of media in enhancing the democratic value is through the use of alternative media and audience participation. It became a space or forum for people to exchange of ideas, opinion and the value is more clearly defined (Becker Rsvelson, 2008). This is how a contemporary media facilitate public discussion and feed into promote a democracy. Nowadays media is fundamental to offer opportunity and right for citizen to associate freely, to educate the citizen about public issue and ideology is one of the factor to motivate the party member ( Clark, 2009). Media play as an active role in public debate and it is generally used to reflect citizen’s criticism, ideas, and agreement through website, vox pop and etc. Basically a democracy requires the active participation of citizen. Ideally, the mass media is used to keep audiences engaged in the government activities. (Coronel 1994). So there’s amount of political news and stories are publish or air in variety media, this allow citizens to appear on screen and speak in their own voices, express their feeling and discuss. (McNair,2000). For example radio has become the channel of choice as it is low cost and more accessible to the public which used to promote grassroots democracy. Besides that, the Vox Pop, is also widely used in television and newspaper stories. The producers will pick randomly and having an interview session with the citizens on the street. Vox pop consider as a form of news coverage, which is help to shape the worldview of citizen and influence the news agenda. (Grab Zhou, Barnett, 1999). As Loviglio (2002) has shown, Vox pop is a news reporting that offer 2 way public communication on a range of topics or issues, so citizens are able to talk and comment about their perception towards democracy. All the public discussion in a move to engage public and listen to the citizen voice. In the new democracies, we expect the media would build a civic culture through a tradition discussion or public debate. Besides, vox pops has make a connection between government and citizen, since everything is mediated through authoritative voice ( Tait, 2001). It is arguable that public opinion has certain impact on democracy. The rise of Vox Pop been also seen as an important element in news organisation, in term to make news more relevant in democracy. However, there’s some argument and critics from other scholars as well. Based on the book by Lewis et. Al. (2005), they argue that vox pop do offer an opportunity for citizen to voice out their political opinion, but at the same time there’s also some limitation. Cottle argues that even citizens participate in the following television talk show, but their voice are not been heard as always because of infrequently allowed to develop their own opinion. Citizen rarely have chance to be â€Å"primary definers† to provide frame work as normally anchor will first introduce the topic, frame the topic area, deliver basic information as a preview.( Hall et al, 1978). Richard Tait (2004) also continue critics that vox pops is just serve as a responds to elitism news programme and more or less similar to an audience participation programme. In addition, Vox Pop allows citizen to participate and openly discuss about their common concern such as political debate, but they are more into expressing their own feeling based on their personal experience and self-interest, rather than the common good or making any improvement. (Richard, 2004). They will speak as a consumer but not as a citizen that attempt to create a better place for their country. Nevertheless Vox Pop still has contribution in shaping a healthy democracy. It create a space for all the political view being expressed and substantive opinion to be seen. Vox pop still serve as a useful media to allow citizen participation and a way to communicate (Lewis et.al, 2005). Besides, democracy is brought about through public debate and it is necessary of mass participation and a requirement for democratic responsiveness to public references (Sartori, 1987). Clearly every political participants attempt to control over the public opinion and structure favourable outcome to their political interest as most of the public are depending heavily on the messages that construed by the media (Callaghan Schnell,2001) So it is important to examine who control the political rhetoric, based on two scholars, Kinders Sanders (1990). They underscored how the media is structuring, interpreting and shaping political issue in democracy. The media has emerge as a more active role in repacking the player’s fame and shaping the policy message. So public debates and public participation is vital to strengthen democratic institutions. In this way, citizen opinion can be considered and taken into account as one of the contributions and effort towards public debating alig ned with democracy. (Coronel, 1994). Public opinion is a tool to control politicians that lead the country, the voices must be heard. Similarly media role is primarily that helping to shape a healthy democracy in pursuit of economic growth. Journalist attempt to influence, educate and inform the citizen through variety type of mainstream media, this is how media role is contributing where politicians can promote their unique policy frames and get the message across to public effectively. (Cohen et.al, 1963). Media are widely used by journalist to convey information to the general public and to expand their visibility in democracy, as the press has tremendous influence (Callaghan Schnell, 2001). A journalist is required with professional skill in news reporting and representing divergent views, which helps to propagate peace journalism. Sen (2001) has argues that free press contribute greatly in assuring an accountable governance. In the other hand, the journalist are challenged by certain limitation and constraints by using media to promote democracy. Some of the unethical journalist practices might causing inability to fulfil democratic function. (Coronel, 1994). At the same time, each media has gatekeepers and they might filter over the information, so not everyone’s voice and opinion can be heard. The gatekeepers often determine the media content and manipulate the messages, so many of the citizen’s opinion has been cut down. Many voices been so frequently been ignored when the gatekeepers who are able to define the story, selecting on what issue is news value and they decide what to publish on the mass media (Clack, 2009). It is pointless if the citizen can only express their criticism in a closed room In doing so, we move on the discussion to citizen journalist as well. Journalist and citizen journalist are two group of people that work together and it is a two way form of news. Citizen journalist is defined as ordinary users who participate and engage in journalist practise which including blogging, photo and video sharing and article posting (Goode, 2009). In other word they are not professional journalist but they are generally associated with internet and they do sharing. Internet allows them to comment or interact with each other, and the most important thing is the freedom from state control. Nowadays media is enveloping and participating in alternative media to promote democracy, and citizen journalist is one of it. (Miller, 2004). A media democracy is basically using mass media to empower all members of society through propaganda and citizen journalism, which citizen can participate themselves in anything by posting or sharing through their cell phone. Based on commenters Pippa Norris (2000), media function mainly as information provider and it helps to improve political engagement that consider as a positive effect of the rise of spin. From this perspective, the deregulation use of media by journalist or citizen journalist actually being beneficial to democracy. However there is scepticism about citizen journalist by some media scholars and critics. They argued that citizen journalists are reducing the quality and credibility by using media. According to Wall (2012), people have perception that citizen journalist is poor for professional news, arguing that they lack of skills to adequately serve as a watchdog on wrongdoing by powerful forces. Besides, the critic argue that citizen journalist might displace the untruth, incomplete, and untrustworthy source in misleading the media audiences. For example: They just publish whatever they see without finding the truth, and attempt to bring popular attention. Another strain of criticism portray that citizen journalism might reinforcing prejudice and muddling the truth, according to Coronel (1994). As a conclusion, media might have certain limitation in rules and regulation but it is possible to see media role has help to enhance in democratic value, in the sense that media is still a powerful and fundamental element which ultimately helps the government in making the democratic system more accountable. Case Study of Diagnosis of Neutropenia Patient A 45 year old physically fit patient was admitted to hospital following several visits to his General Practitioner (GP) where is was complaining of decreased appetite, constipation, fever, chills, headaches, cramping, vertigo and respiratory problems after experiencing some hay fever like symptoms one week ago. He was commenced on Roxithromycin 150mg b.d and his GP ordered blood tests that showed his had neutropenia (low white cell count) (Harris et al 2006, p 1185) and thrombocytopenia (low platelet count) (Harris et al 2006, p. 1704). Two days later he was not feeling any better and the GP ordered a chest x-ray (CXR) which the patient to have bilateral pneumonia he was than admitted to the hospital. This essay will identify important events that took place during the patients admission to hospital and discuss three of these events in detail with contemporary evidence to support the writers discussion. The essay will than look what has been learnt through this case study in relation to future professional practice as a new graduate registered nurse in accordance with the Australian Nursing Midwifery Council (ANMC) competency standards. Day 2 Why did the patient not received oxygen until his saturation got to 70% there is no mention of the treatment plan to or from nursing staff. Later that day the patient was transferred to main ward, the nursing staff raised the issue that the patient needed to be in the intensive care unit (ICU), the patient was reviewed by Respiratory Physician and was decided to not to transfer patient to ICU. This patient required close monitoring due to saturation decrease and as a newly registered nurse we do not have the experience or the time to monitor this patient in a ward environment (ANMC 2006) competency Professional Practice. Patient safety, patient advocacy. Day 5 The patient was noted to be still febrile and was ordered another CXR as the Computed Tomography Scan (CT scan) was not preformed, why had this patient not had the CT scan that was ordered (ANMC 2006) competency Professional Practice. Team communication strategies, chain of command. Later that day it was also noted that the patient had a PR Bleed and a referral was made to the gastroenterologist. No mention of cause or any investigation taken place to assess the PR bleed no blood tests were ordered to determine patients status. Patient advocacy, patient safety. Day 6 Respiratory Physician saw the patient and noted he had severe bilateral pneumonia, possible bone marrow suppression and anaemia of an unknown cause, no communication between medical officers as the patient had a PR bleed the day before and is neutropenic and thrombocytopenic. The Respiratory Physician requested an infectious diseases review. Patient advocacy, conflict management, documentation. Day 7 It was documented again that the patient had low saturations and was febrile. He was seen by the Infectious Diseases Specialist and was ordered more tests and a lung biopsy. The lung biopsy was considered to be of high risk due to the patients condition by a Thoracic Surgeon. If this patient was a high risk why was he not in ICU as requested by the nursing staff? Conflict management, chain of command, patient advocacy. Day 8 The nursing documentation states patient remains very breathless and low saturation is on 12L oxygen via a non-rebreather mask and is still febrile. A BiPAP (bilevel positive airway pressure) was requested to be used the next day but patient was not alive the next day, why was it not used immediately he is on high level oxygen and should have CO2 levels checked by having blood gases taken, a CT scan was ordered of the abdomen as a splenic infarction was suspected. Later that evening the patient was found out of bed with his oxygen mask still insitu but disconnected from the oxygen outlet, why was he trying to get out of bed?, Why was the tubing disconnected was he disoriented due to high oxygen saturations, did he fall out of bed ?, did the tubing need replacing ? This was not documented in the patients notes just handed over to night staff verbally. Patient safety, documentation, team communication strategies. Early Day 9 The night duty staff stated they did not monitor oxygen saturation due to patients request, staff should have explained how important it is for the nursing staff to monitor his oxygen levels, the patient was buzzing several times during night for a urine bottle and to check his oxygen levels, staff gave the patient a urine bottle but left the curtain open, did the patient get up to close the curtains for privacy?. The patient was found on the floor ten minutes later in cardiac arrest, Cardiopulmonary Resuscitation (CPR) was commenced, the arrest team was called and resuscitation continues for 20 and ceased with authority from the respiratory physician. Policy and procedures, Patient safety. First event was that the patient was not placed in ICU as requested by nursing staff. (Patient advocacy) ANMC Competency Standards (2006) Professional practice 1.2 clarifies responsibility for aspects of care with other members of the health care team, recognizes the responsibility to prevent harm. 2.4 Identifies when resources are insufficient to meet care needs of individuals/groups. 2.5 raises concerns about inappropriate delegation with the appropriate registered nurse. The patient has been transferred to the ward after arrangements were made by his GP for admission to hospital following an illness of unknown origin. Tests reveal that the patient has. Bilateral pneumonia. Neutropenia. Thrombocytopenia. Upon presentation it was noted that the patients saturations where low and required 6L oxygen via a Hudson mask and the new graduate registered nurse felt this patient needed to be placed in ICU to receive appropriate care. The newly graduate nurse should have consulted with the with the unit manger about her concerns when the Respiratory Physician refused to place the patient in the ICU this is related to 1.2 of the competency standards by clarifying her concerns and recognizing to preventing of harm to the patient. Patient advocacy is not new to nurses, nurses assume they have an ethical obligation to advocate for their patients (Negarandeh et al 2006). Advocacy can be defined as the act of arguing in favor of something. In terms of nursing, advocacy can be seen as the process we engage in when we speak on behalf of our patients when they are incapable of doing so themselves (Spenceley et al 2006). Advocacy has been described in ethical and legal frameworks and, more recently, as a philosophical foundation for practice (Negarandeh et al 2006). It has also been described in terms of specific actions such as helping the patient to obtain needed healthcare, defending the patients rights, assuring quality of care, and serving as a liaison between the patient and medical officer (ANMC 2005,Code of Ethics), (Spenceley et al 2006). When nurses and new graduate nurses advocate for patients, they face certain risks and barriers associated with the settings within which they work (Negarandeh et al 2006). Therefore, there is always the possibility that attempts to advocate for a patient can fail, and that nurses can experience many barriers when addressing choices, the rights, or welfare of their patients (Negarandeh et al 2006). As a new graduate nurse this was an important event to advocate for their patient as it was quite evident that this patient needed extensive nursing care that is not available in the ward situation, the nurse needed to gather evidence to suppor t their concerns and present it to the Physician and nurse unit manager (Spenceley et al 2006). The second event was why did the patient not attend the requested CT scan (negligence, communication). ANMC Competency Standards (2006) Provision and Coordination of Care 7.1 Effectively manages the nursing care of individuals/groups. It is the registered nurses responsibility to make sure that all tests and orders by the medical officer are carried out. The registered nurse could be seen to be negligent of her duty by not making sure the patient had received his CT scan according to the treatment plan. The legal definition of negligence means breach of duty. Standards of care in nursing generally mean those practices that a reasonably average nurse would use. So a good nurse knows and understands ethics in the medical field and strives to provide excellent quality of care in order to avoid negligence. However, mistakes, which will happen, do not necessarily mean negligence has occurred (Staunton Chiarella 2008). Examples of breach of duty, which may be considered negligent under certain circumstances may include doing something which a reasonably average person would not do, or the failure to do something which a reasonably average person would do, under circumstances similar to those shown by the evidence. It is the failure to use ordinary or reasonable care, (Staunton Chiarella 2008). Inadequate nursing skills or attention to tasks may result in a suit of negligence against a nurse who habitually fails to provide approved standards of care. Such incidents include, but are not l imited to, constant medication errors, failure to follow policy or orders and improper use of equipment (Hughes 2008). The legal review of a nursing negligence requires proof that injury was done, and that it was the result of the nurses care or lack of. Negligence refers to the act of doing something or refraining from doing something that any other reasonable medical professional would do or refrain from doing in a similar situation (Staunton Chiarella 2008). It goes without saying that every situation is different, and that is where the law becomes somewhat cloudy. However, when reviewing a nursing negligence case, assumptions and circumstantial evidence are taken into account to determine if there was negligence (Staunton Chiarella 2008). There are five main elements in a nursing negligence, and all elements must be proven in order for a case to be valid. If one or more of the elements is not present, the case may be hard to prove, (1) the nurse had a duty to perform, (2) the appropriate care was apparent in the situation, (3) there was a breach or violation of care, (4) there was an injury proven to result from the nurses negligence, and (5) there is proof that damages occurred as a direct result of the situation (Staunton Chiarella 2008). Her husband had just passed away and she was grieving his death. Van Gogh asked her for her hand in marriage but she denied. Van Gogh talked to her family and they explained to him that she didn't want that relationship. His family was very disgusted with him for his actions. And as a lot of people know, Van Gogh is the oldest child in their family, but actually, his parent's had a little baby before him, the baby died the same day Just one year before Van Gogh was born. At the age of twenty-seven, Vincent decided to devote his life for art because Vincent believed in the power of art. To him, a work of art, whether drawing or painting, had as striking an â€Å"effect† as a live sermon or a letter†( Euchre 17) . Van Gogh started his work of art with drawing.Vincent sent Thee, is brother, some of his drawings such as Au Carbonate (At the Coalmines), Miner Shouldering a Spade or Men and Women Going to the Mines. He drew everything he saw in everyday life. The miners and weavers usually appeared in his drawings. In the letter to his brother, Thee, he told him about his sympathy for these people :† I should be very happy if someday I could draw them, so that those unknown or little- known typ es would be brought before the eyes of the people†(20). In summer of 1882, he started paintings in oils.Vincent often paints † the simplest, most miserable, cost scorned objects† such as old shoes or † loamy' potatoes. (Huge 61). Vincent painted with dark and sorrowful colors. At the end of 1882, he experiments with lithography. He created a series often graphic works: nine lithographs and one etching. In the letter to his brother Thee, Vincent told him about drawing: Sorrow is one of Van Sago's lithographs. The special thing about this lithograph is that it was originally a painting about a naked prostitute † sitting with flabby breasts, heavy stomach, thin and stiff hair and weeping with her head on her knees† (Huge 6).Vincent also decided to do a lithograph based on the painting The Potato Eaters. The Potato Eaters is his first major work. It depicted five miserable family members, who were peasants, gathered around the table, eating their pota toes in the end of a day. Vincent wanted to attain the moral value in this painting: † I have tried to emphasize that those people , eating their potatoes in the lamplight, have dug the earth with those very hands they put in the dish, and so it speaks of manual labor, and how they have honestly earned their food† (Euchre 88).Van Sago's only etching, L ‘Home  ¤ la Pipe: Portrait du Doctor Cache,depicted Dry. Paul Cachet, a doctor and friend of Van Sago's who took care of him in the final weeks of his life. In 1886, Vincent moved to Paris, where he met his brother Thee, and he also made friends with Gauguin, Pissarro and Serrate. He discovered the French Impressionist. However, through Monticello, he â€Å"discovered both flowers and color† (Huge 63). And through his new friends, he brightened his palette. The Million De la Galatea is yellowed in tone but still muted in spite off pale sky.The hill of Montmartre was treated more conventionally; the painting was dominated by green and yellow ( Euchre 114). During 1887, Vincent created more than twenty- two self- portraits. The reason for this large amounts of his self-portraits was as though he wanted to rediscovered himself physically after he changed his style in art . Another reason is that Vincent Van Gogh was short of money, therefore, he became his own subject: â€Å"l purposely bought a good enough mirror to work from myself, for want of a model†.Self -portrait with a Straw Hat painted in the summer of 887 † show the artist's awareness of Neo-elementariness technique and color theory† ( The Metropolitan Museum of Art), † the background is sketched in by meaner of very widely spaced, comma- like strokes† ( Euchre 1 15). In 1888, he moved to Arles. Vincent was affected by the strong sunlight and he became † the prey of the sun† , † he burst out with spring, reached his highest point during summer and discovered July and the flame's p lenitude†(Euchre 165).At this point of time, his style changed immensely , and it was greatly impacted by the work of the Impressionists and Neo- Impressionists. He started to abandon somber palette and experiment with lighter colors such as red, yellow, orange, green, blue. We can easily see this change in his paintings such as The Pear Tree in Blossom, The Angle's Bridge, Boats at Anchor, Market Gardens, The Suave, A Walk in Arles, The Cafe at Night†¦ Etc. Besides, Vincent also developed his own unique style of painting. Vincent also experimented with the broken brush strokes of the Impressionists and hugely influenced by the Japanese prints.In 1887, he usually went to the shop of Samuel Being, a dealer of oriental art. He purchased many Japanese prints from Being. After that, he organized an exhibition of Japanese prints at the cafe Lee Tambourine. The Japanese prints were used in the background of the painting Portrait of Peer Tangy by Vincent. In the painting Still L ife with Plaster Statuette,† the plane in which the plaster statuette is placed crosses the picture surface vertically from one edge to the other, for shaking all the rules of illusionist perspective for the† plunging† approach of the Japanese†( Euchre 130).In the Woman at Lee Tambourine painted from the beginning of 1887, â€Å"the technique is Impressionist, but the background is treated in Japanese fashion, that is divided into parallel down in vertical stripes† (132). Between the years 1886 and 1889, Vincent painted more than over 30 self- portraits, reflecting his current pursuit of integrative color contrasts and a bolder composition. His collections of self-portraits place him among the most productive self- portraitists of all time. These portraits not only marked changes in his painting technique but also a reflection of his psychological world.The painting â€Å"Self Portrait with Pipe† was painted in 1886, before Vincent moved to Pari s. This is the typical kind of Vincent earlier painting style, using melancholy colors, which are characteristic of traditional Dutch painting. In 1887, Vincent was trying to use Pointillism for his â€Å"Self Portrait†. Pointillism is a painting technique which was created by the artist Georges Serrate. However, Vincent failed at imitating † Caesura's patient and analytical approach to the technique† ( The History in Self Portraits). Another self portrait of Vincent Van Gogh is † Self Portrait with a Gray Felt Hat†.This portrait showed that he reached the peak of his new painting technique. He also applied brush strokes of different contrast colors such as blues, oranges, yellow, white, lilac, sky blue, and green. In June 1888, Vincent was interested in the night of Saints- Maries in Arles that he decided to paint the beautiful view of the night. It was the † Night Cafe† which Vincent stated in the letter to Thee :† equivalent, thoug h different, of the † Potato Eaters† and † In my picture of the † Night Cafe† I have tried to express the idea that the cafe is a place where one can ruin oneself, go mad or commit a crime†( Euchre 164).Then, Vincent had his second night study : â€Å"Cafe Terrace by Night†. In the letter that Vincent sent to Wilhelmina, he expressed his pleasure to his paintings: Here you have a night picture without any black in it, done with nothing but beautiful blue and violet and green, and in these surroundings the lighted square acquires a pale sulfur and greenish citron-yellow color. It amuses me enormously to paint the night on the spot. They used to draw and paint the picture in the daytime after the rough sketch. But I find satisfaction in painting things immediately.Of course it's true that in the dark I may mistake a blue for a green, a blue- lilac for a pink-lilac, for you cannot rightly extinguish the quality of a hue. (Euchre 164) In Octobe r 1888, Gauguin came to Arles and lived with Vincent. Gauguin was Vincent friend. He was famous for creating painting in many different styles, including Impressionism and Primitivism. When being together, they painted paintings and discussed with each other about different technique. Next month, under Gauguin urging, Vincent experimented with painting from memory.Vincent started to imitate Gauguin technique of painting from memory during this time that resulted in his painting becoming less realistic and more attractive. Vincent deliberately used colors to capture moods, rather than using colors realistically. Their first encounter in painting took place at the Alleyways. In December 1888, Van Gogh did two chair paintings: Vincent Chair and Gauguin Armchair. In Vincent Van Gogh- Art, Life and Letters, Euchre said Vincent chair † is a creature of light.The intensity of the yellow note, emphasized by the vibrant blue of the contour , marks the paintings of the future painted â €Å"in such a way that everybody , at least if they have eyes, would see it†. Whereas the elegant armchair is bathed in full chiaroscuro'(202). However, their enthusiasm for each other and art faded away, and soon replaced by quarrels. Vincent had less and less confidence in Gauguin. He wrote Thee: On various occasions I have seen him do things which you and I would not let ourselves do, because we have consciences that feel differently about things.I have heard one or two things said of him, but having seen him at very, very close quarters, I think that he is carried away by his imagination, perhaps by pride, but†¦ Practically irresponsible† ( CTD. In Euchre 202). About Vincent, he did not eat every day because he was short of money. Despite an empty stomach, he stayed for hours working under the sun. (Huge 80) . One time, when Vincent saw the portrait of Vincent Painting Sunflowers which was hidden by Gauguin , he could not recognized himself and reacted violentl y to Gauguin.When they went to a cafe, Vincent suddenly threw his glass of absinthe at Gauguin head. When Gauguin decided to leave Arles, Vincent tried many ways to make him stay. He had been through a difficult time to face with Gauguin threatened departure. He got up during the night to make sure Gauguin was still there, he used absinthe and bacon, since † the only thing to bring ease and distraction, in my case and other people's too, is to stun oneself with a lot of drinking or heavy smoking†(Euchre 211).One day, when Gauguin was walking in the street, Vincent threw himself at him with an opening razor in his hand. However, he did not attack Gauguin, he ran away. Back in his room, he cut off the lobe of his ear with the razor. He cleaned his ear and put it in an envelope and gave the doorkeeper his ear. He was discovered by the police and hospitalized at the H ¶tell-Died hospital in Arles. Gauguin left immediately for Paris, choosing not to visit Van Gogh in the ho spital. Then they never met each other in person again.Two weeks after it happened, he painted his self- portrait: Self -Portrait with Bandaged Ear. This portrait is considered as â€Å"the calm after the storm. He takes a quiet and detached look at himself and expresses a feeling of renewed hope through his bright colors and simplified drawing. A clue to the origin of this bold style is found in the Japanese woodblock print on the wall behind him†( The History in Self Portraits Para. 13). Van Gogh then started to alternate between fits of madness and lucidity and was sent to the asylum in Saint- Remy for treatment.Van Gogh left Arles on 8 May. In mid-June, Van Gogh produced his best known work: Starry Night. In 1890, he shot himself in the chest by a revolver. During his career, he only sold one painting. Van Gogh became famous only after his death because of his innovative art that had a strong influence on the artists of the next generation.
